- Picture ID
- Insurance card
- Referral from your primary care physician, if applicable
- New Patient Forms: In order to reduce wait time at your first appointment, we ask that you have these forms completed prior to coming to your first appointment. We will email you these forms or you may click to download here.
- If you have any imaging results from an X-ray or MRI, please bring in a disk, the films, and/or the written results report.
Non-Payment
We ask that you contact the billing office with any questions or concerns regarding your balance due. Payment for any balance greater than 45 days will be expected at the time of subsequent visits. If your account is over 60 days past due, you will receive a letter stating that you have 15 days to pay in full. Failure to pay the balance by 90 days may result in referral to collections or possible discharge from the practice. If you are inbetween insurances, or have no insurance, please contact our billind department prior to your appointment.

As a courtesy to you, we will be happy to bill your insurance company for the services rendered on your visit date. In order to do so, it is essential that you bring a valid insurance card with you to each visit and communicate with our reception staff if there are any changes in coverage or billing information.

Co-Payments and Deductibles
All co-payments are due at the time of service. Deductibles are based on your contract with the insurance company and are your responsibility to pay. We accept cash, check, or credit card (Visa & MasterCard).

Please be aware that not every insurance policy covers every service rendered, including physical exams. Because of the vast array of insurance policies and benefits, it is impossible for our office to know every service your individual policy covers. Insurance
We participate with most insurance plans. If your insurance plan is not listed, please call us at (240) 362-7128. If you are not insured by a plan with which we do business, or if you have no insurance, payment in full is expected at the time of your visit. If you are insured by a plan we participate with, but do not have an up to date insurance card/information, payment in full is required at the time of your visit.
